Parental / Guardian Consent (Version 1)

I, [guardian_name], parent or legal guardian of [athlete_name] (date of birth: [dob]), hereby give explicit consent to the Maharashtra Aquatic Association (MAA) to collect, store, and process my child's personal data through the SwimSetu platform for the purpose of swimming registration, district affiliation, and related sporting administration.

I understand that this includes: name, date of birth, gender, district, parent names, contact information, address, and upload of identity documents (e.g. birth certificate and Aadhaar card). Data will be stored on servers located in India (Mumbai and Bangalore).

I understand that MAA district and state administrators will review this registration; that an official MAA ID is issued only after final approval; that an approved ID is valid for one year and may be renewed within the renewal window described in the Terms of Service; and that I may withdraw consent or exercise data rights by contacting the Grievance Officer at [GRIEVANCE_EMAIL], subject to retention requirements for approved sporting records.
